脑-肠轴失衡为单纯性肥胖发生的重要原因,而免疫炎症因子的激活又是脑-肠轴失衡的中心环节,免疫反应发生发展中起到关键作用的是TLR5/REG3γ信号通路。因此针刺对于此信号通路的干预是重要着眼点。穴位埋线疗法用于治疗单纯性肥胖在临床上已广泛开展,其治疗的有效性已得到认可,这是中医学传统理论与现代脑-肠轴理论成功有机结合的产物。通过整理穴位埋线疗法治疗单纯性肥胖所涉及的中医学病因病机、肠道免疫应答反应以及调节TLR5/REG3γ信号通路等主要机制,结合针刺对抑制TLR基因、增加REG3γ基因表达的积极效应,对穴位埋线疗法治疗单纯性肥胖提出新的科研思路,以期为今后的相关研究提供参考依据。

Brain-gut axis imbalance is an important cause of simple obesity, and the activation of immune inflammatory factors is the key link of brain-gut axis imbalance. TLR5/REG3γ signal pathway plays a key role in the occurrence and development of immune response, therefore, acupuncture is an important focus for the intervention of this signaling pathway. Acupoint catgut embedding has been widely used to treat simple obesity in clinical practice, and its effectiveness has been recognized, which is the product of the successful combination of traditional Chinese medicine theory and modern brain-gut axis theory. By organizing the main mechanism of acupoint catgut embedding in the treatment of simple obesity, including the etiology and pathogenesis of traditional Chinese medicine, intestinal immune response and the regulation of TLR5/REG3γ signal pathway, a new scientific research approach is proposed for the treatment of simple obesity through acupoint catgut embedding, which combined with the positive effects of acupuncture on inhibiting TLR genes and increasing REG3γ gene expressions. It is expected to provide reference basis for future relevant research.
